I don’t understand how if this is truly the Black Plague, like from the 14th and 15th centuries in Europe, why anybody is thinking it would spread like wildfire. I thought it was only spread by rat bites.

Third world countries have poor sanitation and contaminated drinking water. Rats have fleas that bite people and spread plague. Airline travel spreads to other African countries as people carry fleas in their luggage or in their clothing. Then they board buses, trains when they leave the plane. Probably the airlines avoids to clean the inside and fleas may be in the seats or carpet.
